you download, print and fill out the membership application below, please sign
it and return it to us via Postal Mail or Fax to the address below.
Attach a
clear copy of your valid Driver's License or State ID, a clear copy
of a Birth
Certificate or Social Security Card, any document establishing
your Proof of Residence, a recent Pay Stub, and a Check
or Money Order for at
least the minimum required opening deposit of $6 for savings account only or
$31 for savings and checking.
Please print, complete, sign and mail the
above Membership Application and include the following:
- $6 Deposit (minimum)
- Photocopies of ALL of
the following documents for EACH person on this account.

1.TWO FORMS OF IDENTIFICATION –
A) a photocopy of a valid driver’s license or state ID issued by your state
of residence
B) a photocopy of a birth certificate, a social security card, military ID or
a voter’s registration card issued by your state of residence.
2. PROOF OF RESIDENCE – any one of these are acceptable
providing they are registered at the address on your driver’s license or
state ID:
A) a phone number published in your name
B) a recent residential phone bill
C) a recent utility bill (e.g. water, gas, electrical, cable, etc.)
3. PROOF OF MEMBERSHIP ELIGIBILITY –
A) if you are joining based on employment; please provide a valid employee ID
or a recent pay stub by mail.
4. MINIMUM INITIAL DEPOSIT REQUIRED – To open a Savings
Account only please enclose a check or money order for $6. To open a Savings
and Checking Account please make the amount for $31. If you are applying by mail,
please do not send cash - send only a check or money order.
